Please click on the above link. The form will ask you questions to ask yourself and your clients. If you answer "Yes" to any question on the form you must call your client's care coordinator immediately and they will help you.
YOU MUST CLICK ON THE LINK, ANSWER, AND SUBMIT THE FORM BEFORE EVERY PATIENT VISIT.

Dallas County has extended its Stay-At-Home order to help mitigate the spread of COVID-19. Agape Home Health and Hospice will continue to be open and serve the Dallas/Fort Worth Metroplex during this time.
Home Health and Hospice are essential services.

We are monitoring the COVID-19 situation. If you or your client have recently been out of the country or are experiencing fever, coughing, or shortness of breath, please notify your care coordinator. Please not that our offices are open during normal business hours and we are continuing business as usual and will continue to provide care.
We are following CDC recommendations.
The company will provide updates regarding COVID-19 and any other potential emergencies that pertain to Agape field staff at this website.
